Sign Up Information

• For a limited time, Saskatchewan residents can sign up to be MedicAlert subscribers and received their first year service plan and a MedicAlert identification at no cost by using the MedicAlert Registration Sheet*
• When completing the form, please indicate which ID you would like at the bottom of the form (available options can be found on the separate attachment)
• Completed forms along with an updated photo of the registrant can be sent in by one of the following:
